Roadmap: repeat gates clear themselves from your own precedent

Decision memory today prefills a recommendation from your past answers. Next: a precedent with a proven acceptance record clears the repeat gate itself, with the precedent cited in the audit trail, so you are never asked a question you have already answered. Promotion is gated on measured prefill acceptance rate, never on similarity alone. Part of the Human Gates 2.0 line.

Progress: precedent-based auto-clear shipped as a fleet-wide switch, 5dive task precedent on|off (defaults OFF for safety, surfaced in 5dive doctor). With it on, a resolved tier-1 gate that matches proven human precedent clears without a ping, with the precedent recorded in the audit trail. Decision-memory prefill (a recommendation from your past answers) shipped earlier.

Still to do per this issue: gate the promotion on a measured prefill-acceptance rate rather than the operator toggle, so a precedent only starts self-clearing once its acceptance record proves out.
