test: 100% line/branch coverage; docs: 100% docstring coverage

- Add docstrings to all public functions, methods, and classes across the package
- Add unit tests for mcp_server.py (was 0% in-process coverage; subprocess tests
  don't count — added direct _handle/serve/main unit tests)
- Add tests for installer.py: resolve_target, resolve_skill_target,
  install_all_skills, resolve_marketplace_target, extract_frontmatter,
  render, find_markers, overlay, _cli_entry edge cases
- Add tests for all provider stream edge cases: URLError, [DONE] break,
  JSONDecodeError, non-data: lines; plus ollama _validate_ollama_base
- Add tests for cli.py error paths: bad repo, resolve_bundle failure,
  cmd_init --full scan failure, cmd_mcp_server, stream ProviderError, TTY info
- Add tests for config.py: _require_type, _coerce_str_tuple, _parse_api_key_env,
  _to_int validation errors
- Add tests for doctor.py: canonical missing, copy missing, installed-destinations
  OSError/markers/no-markers/marketplace cases, _skill_mode_active, ConfigError path
- Add tests for check.py: CHANGELOG.md skip, ignored path, unreadable doc, _path_ignored
  ValueError branch, _current_version OSError
- Add test_bundle.py for bundle edge cases (bad repo_arg, cwd walk-up, package fallback)
- Mark 5 genuinely unreachable lines with # pragma: no cover:
  PackageNotFoundError fallback in __init__.py, urlparse-never-raises in ollama.py,
  YAML-keys-always-strings in config.py, _coerce_str_tuple(None) in config.py,
  __main__ block in mcp_server.py

Total: 304 tests pass, 100% line coverage
