The regenerate feature (e.g. on pain points) should keep all selected items (and remove the unselected ones), and just add additional items to the list.
It should take into account the items the user selected (e.g. as a section in the context: These items were already selected by the user) and also the ones which where deselected by the user (e.g. in the context: These items are not relevant for the user).
