Gate for TASK-311. The head moved twice while I measured: the ask names e2630c5a02e01f2f4da1ce46cb93f107da2f4b4f, and the live head is now 3d286d83f21a58a768441f2130f5863161a7e7e2 (two commits later, 10:22 and 10:25Z). This verdict binds the live head; where a finding was measured at the asked head I say so, and server.ts is untouched between them, so those measurements carry.
One scope limit first: TASK-311 is not on this pod's board, so I could not read its shape ruling. I gated the five surfaces the ask names plus what the code claims about itself. If the row carries a ruled item I have not covered, name it back at me and I will measure that too.
Verdict: HOLD. Three items in the rate limiter the last commit added, and three from the signed-boundary pass. The core signature design is sound and I have listed below what I confirmed rather than assumed.
1. The limiter exempts invalid signatures, so the path an attacker controls is still unlimited
skip returns true when the signature does not verify, so an unverified request never touches the limiter's store. Driven as production against the real route:
| request | result |
|---|---|
| verified delivery | 204, RateLimit-Limit: 3000 |
| bad signature | 401, no RateLimit headers, bucket untouched |
| verified delivery after two bad-signature requests | bucket moved only for the verified one |
So what is now rate-limited is GitHub's own authenticated traffic; a flood from anyone without the secret is bounded by nothing but the 128 KB body read. That is the exact shape of the alert the commit answers — "This route handler performs authorization, but is not rate-limited" — and CodeQL now reports success with zero open alerts for this PR, so the scanner's pattern is satisfied while the property is not. Nobody will re-raise it, which is why it belongs in a verdict.
The comment explains the intent — "Unauthenticated traffic must keep the same empty 401 even when a source bucket is full" — and that goal is compatible with counting: count every request, and on overflow keep returning the empty 401 instead of the 429. Then the response stays indistinguishable and the bucket bounds the flood.
2. The limiter is in the chain twice, so every delivery costs two hits
router.post('/', githubGigRateLimit, verify, githubGigRateLimit, handler). Measured on consecutive verified deliveries: remaining goes 2998 → 2996 → 2994. The advertised RateLimit-Limit: 3000 is therefore an effective 1500 deliveries per minute per source, and each delivery computes three HMACs (the first skip, the explicit check, the second skip) where one would do.
3. skip exempts NODE_ENV === 'test', so no test can witness the limiter
Mounted under NODE_ENV=test, a verified delivery comes back with no RateLimit headers at all — the limiter is skipped end to end. The backend suite runs under that env, so neither the window, the max, the key generator nor the 429 handler can be exercised by any arm, now or later. If the exemption exists to keep other suites from tripping the bucket, a per-test override of max or an injected store does that without making the whole feature untestable.
4. An unauthenticated oversize body gets an HTML error page, before verification
Measured at the asked head, unchanged at the live one since server.ts did not move: a 200 KB body with a valid signature returns 413 with a 3,333-byte HTML PayloadTooLargeError page, produced by express's default handler before the route runs. Two consequences. The route's claim that "signature failures have one indistinguishable response" does not survive an oversize probe, which anyone can send without a secret; and an API route answers HTML. Separately, GitHub delivers payloads far above 128 KB, and a 413 is a failed delivery whose retries fail identically — so a large legitimate event is lost rather than throttled. A type-matched raw parser with a higher ceiling, or an explicit empty 413, would close both.
5. The PR's own webhook harness omits the production limit
github.webhook.test.js mounts express.raw({ type: 'application/json' }) with no limit, while server.ts:227 mounts it with limit: '128kb'. That is why item 4 is invisible to its five arms: the harness cannot produce the one status the production mount can.
6. A processing failure after the receipt loses the card permanently and silently
Driven at the real route: receipt written, handler throws, the 204 has already gone out. GitHub does not retry a 204, and a manual redelivery carries the same delivery GUID, which the permanent receipt refuses — so the card is gone for good. The only trace is console.warn('[github-gig] bound delivery processing failed'), with no delivery id, event type or pod. The comment's rule — no payload text or secret in a log — is right and does not forbid our own identifiers; adding those three makes the loss diagnosable.
What I confirmed rather than assumed
Driven at the real route, with the production mount: a body re-serialised from the same object 401s (the signature is bound to bytes, not to JSON); a missing signature, a wrong secret and an unset GITHUB_GIG_WEBHOOK_SECRET each 401 with an empty body and no binding lookup; a non-JSON content type fails closed; the flag off returns 204 with no lookup at all; an unbound repository returns 204 and writes no receipt; a replayed delivery id returns 204 and does not post twice; a receipt write failing for any other reason returns 503, so GitHub retries.
The replay guarantee rests on the new collection's unique index existing, so I checked rather than trusted it: config/db.ts passes no autoIndex: false and the backend sets none globally, and the live cluster shows schema-declared unique indexes present on another recently added collection. The receipt will be unique. (A migration script's comment asserts "the backend turns mongoose's autoIndex off" — that is misleading, and someone will rely on it.)
Outsider text: the card is built from a closed element catalogue — an event badge, #number, owner/repo, a hardcoded https://github.com/ link and a dictionary-mapped review state — with a 32 KB bound, and no title, body or review text enters it. owner/repo/number are regex-pinned before use, so the href cannot leave github.com. Merged-PR completion is pinned to the exact href and the bound pod, and the binding route is auth, adminAuth, so no plain member can point a repository at a pod.
Correcting the ask on one point: Detect secrets did not skip its scan at this head. The run for the asked head (39583678883) and both later runs are green with every step succeeding, including "Install gitleaks" and "Run gitleaks". Whatever connection reset you saw was superseded. Several other checks show cancelled on the live head — those are superseded instances, not failures, and the newer ones are green; Test & Coverage was still running when I filed.
One note, not a finding. The bind route accepts any podId that Pod.exists finds, including an agent-room or agent-dm pod. The DM membership guard would then refuse every card the handler posts, and item 6's catch swallows the failure — so an admin who binds a repository to a DM pod gets silence rather than an error. Checking the pod type at bind time costs one field.

Re-gate at 6833d16c6142d33575defefce9caa0926b5ab98e, the live head when I filed. The ask names 1cbe5542; one more commit landed while I measured, and it matters, so this verdict binds the newer head and I re-ran the affected probes there.
Verdict: PASS. All six items from my HOLD are closed, each measured at this head rather than read off the summary, and each new arm bites when I reintroduce the defect it covers.
The six, measured
1. The limiter counts unauthenticated traffic, and the response stays uniform. skip is gone. With a bucket of 2 in front of the real chain: three bad-signature requests return 401, zero-length, with no RateLimit header at all, and a valid delivery on the now-full bucket returns 429 — so unauthenticated traffic is counted, the flood path is bounded, and no binding lookup happens while the bucket is full (0 lookups). The overflow split res.status(signed ? 429 : 401).end() is witnessed by an arm that exhausts createGithubGigRateLimit(2); exporting the factory is what makes that arm possible.
And the last commit closed something I had measured and failed to flag. At 1cbe5542 the limiter ran with standardHeaders: true, so a bad-signature 401 carried RateLimit-Remaining — my own probe output printed BAD SIGNATURE status=401 remaining=2997, which both breaks the uniform-response property the route's comment claims and hands an attacker the bucket state. I read that line as evidence of counting and did not notice it was also a leak. 6833d16c sets standardHeaders: false and adds an arm asserting the three 401s carry no ratelimit-limit; measured at the new head, a verified 204 on the real route carries no ratelimit-* header either. Caught by the builder, not by me.
2. One hit per request. Consecutive deliveries read 2999 → 2998 → 2997 → 2996, a single decrement each. There is also an arm asserting the route mounts its limiter once, and a double mount reddens it.
3. No test-env skip. The run above happened with NODE_ENV=test and the RateLimit headers were present, so the limiter is live under the suite and its behaviour is testable rather than exempt.
4. The parser is shared and the 413 is empty. githubGigRawBody and githubGigRawError are exported and mounted by server.ts, so the harness and production now use the same parser — which was the reason item 4 was invisible before. A 30 MB body returns 413 with a zero-length body: no HTML error page. The ceiling is 25 MB, matching GitHub's own delivery limit, so a large legitimate event is no longer dropped at 128 KB.
5. The handler completes before the ack, and a failure releases the receipt. On a throw: 503, with deleteOne called for exactly that deliveryId, so GitHub's retry can reprocess instead of meeting a permanent receipt. On success: 204 after completion, no delete. The warn line now carries deliveryId, eventType and podId — our own identifiers, no payload text.
6. A DM pod is refused at bind time, with agent-room and agent-dm rejected 400. I read the route rather than the diff fragment here, because the fragment looked as though the Pod.exists → 404 check might have been replaced: it was not. findById(...).select('type') is followed by if (!pod) return 404 at :287 and the type check at :288, so a missing pod still 404s and only a real DM pod gets the 400.
The arms bite
One variable each, tree verified clean after every arm:
| mutation | arm reddened |
|---|---|
re-add skip on an invalid signature |
counts invalid signatures but returns empty 401 on overflow |
| overflow handler always 429s, even unsigned | the same arm |
| receipt not released on a handler failure | a handler failure releases the receipt and returns 503 |
| 413 handler lets the HTML page through | the production parser returns an empty 413 above its ceiling |
| mount the limiter twice | the production route mounts its source limiter once |
19 passed across github.webhook, integrations.githubGigBinding and githubGigService.
What carries unchanged from the first verdict
Everything I confirmed at 3d286d83 and e2630c5a still holds, and this commit does not touch it: the signature is bound to raw bytes (a body re-serialised from the same object 401s); a missing signature, wrong secret or unset secret each 401 empty with no binding lookup; the flag off returns 204 with no lookup; an unbound repository writes no receipt; a replayed delivery does not post twice; a receipt write failing for another reason returns 503; the receipt's unique index will exist because nothing disables autoIndex; the card is built from a closed element catalogue with regex-pinned owner/repo/number and a hardcoded https://github.com/, so no outsider title, body or review text can enter; and merged-PR completion is pinned to the exact href within the bound pod, behind an admin-gated bind route.
The scope limit from my first verdict stands: TASK-311 is not on this pod's board, so I gated the five named properties plus the code's own claims. If the row carries a ruled item I have not covered, name it and I will measure it.

Delta re-gate at 6871a118c9a9ac4627d3ee11d4dffbe83ccf3aff, the live head when I read it and again at filing. My PASS at 6833d16c (review 5598480263) is the baseline.
Verdict: PASS. The delta is test-only and the new instrument is sound in both directions — it sees a header-suppressing limiter, and it still fails when one stops counting.
Test-only, checked rather than accepted. One file changed between the two heads: backend/__tests__/unit/middleware/rateLimitIpKeySeparation.test.js. Diffing the heads with __tests__ excluded is empty, so the product tree is identical and every measurement in the baseline verdict carries without re-running. (I re-ran the three webhook suites anyway — 19 passed — because the changed file sets process.env.PG_HOST and mocks two db modules, and I wanted the neighbours checked rather than reasoned about.)
The instrument change is the right shape. TASK-126's census walks the loaded app and identifies limiters by own properties, so the GitHub limiter entered it automatically — and once standardHeaders: false landed at 6833d16c, a header-reading probe could no longer see it. The two ways out were an exemption entry or a better instrument; this takes the second. remainingOf now prefers x-probe-remaining, which the stub sets from req.rateLimit.remaining, falling back to the public headers and then to null.
Both directions measured, because an instrument fix is exactly where a vacuous pass hides:
| mutation | result |
|---|---|
the GitHub limiter never counts (skip: () => true) |
"every walked instance counts and separates" reddens — so the census genuinely reaches that instance, and "no exception for GitHub" is substantive rather than a claim about an absent exemption |
the stub stops setting x-probe-remaining |
the new headerless arm reddens, and so does the full walk — the control is not passing through the old header path, and the walk itself witnesses why the fix was needed |
The second row is the one worth keeping: the fix's necessity is asserted by the same walk it repairs, so a future revert of the stub change cannot pass quietly. And remainingOf's null fallback keeps the no-limiter case failing — null === null - 1 is false — so a route that loses its limiter entirely is still caught.
6 passed in that suite at this head, matching the ask.

Exact-head gate at 1fb87289a11fad2643972be391e74bfb11d5e32a, re-resolved immediately before filing. The ask named 310944388; this head adds the ops-runner env-parity entries on top of it, and backend/, frontend/ and docs/ are byte-identical between the two, so the suite and mutation results below were taken at the earlier head (tree 214e908d) and carry, while Helm was re-measured here. main is 71bd369d.
Verdict: HOLD, two items. Everything else I checked holds, and the two rebase resolutions you asked me to attack are sound in the frontend and wrong in one line of Helm.
HOLD 1 — the gig ExternalSecret key does not follow $keyPrefix. api-keys.yaml adds key: commonly-dev-GITHUB_GIG_WEBHOOK_SECRET; its 62 prefixed neighbours render per environment. Rendered on the main-merged tree, with a control in the same render:
| values file | gig key | control (App installation id) |
|---|---|---|
values-staging.yaml |
commonly-dev-GITHUB_GIG_WEBHOOK_SECRET |
commonly-staging-github-app-installation-id |
values-dev.yaml |
commonly-dev-GITHUB_GIG_WEBHOOK_SECRET |
commonly-dev-github-app-installation-id |
Seven of 69 keys skip the prefix; the other six are a deliberate shared class (commonly-cloudflare-*, commonly-deepseek-*, commonly-github-pat, commonly-git-user-*). This is the only key that is environment-scoped by name and fixed to one environment by template. It is inert at this head — no values file sets githubGig.fromSecret: true — so the cost falls on the first enablement outside dev: either the wrong environment's secret is used, or ESO cannot resolve the key and the env block, which unlike its neighbours carries no optional: true, stops the pod. #2430 introduced githubPat.enabled three commits ago for the adjacent reason, which makes the convention here load-bearing rather than cosmetic. One line: key: {{ $keyPrefix }}github-gig-webhook-secret, in the lowercase-hyphen form its prefixed neighbours use.
HOLD 2 — the signature test covers the format guard, not the comparison. The property this endpoint rests on is that a signature which is well-formed but not computed from the configured secret is refused before any parse, binding read or receipt. The behaviour at this head is correct — I measured it directly with a throwaway probe, which passes. What is missing is the arm: the suite's only negative input is sha256=0000, which the /^sha256=[a-f0-9]{64}$/ test rejects on its own, so no test distinguishes "well-formed" from "correct". I confirmed that by mutation rather than by reading: a change that leaves the format check intact and makes the comparison unnecessary keeps all 12 tests green, and reddens the probe. Please add the wrong-secret case beside the existing one; it is four lines, and it is the difference between the format guard being tested and the verification being tested.
Third item, the same class, your call on whether it blocks. GITHUB_GIG_ENABLED is the only control before a binding exists, and no test exercises it: every test sets it to 'true', and replacing the gate with a constant leaves 12/12 green. One arm asserting a signed delivery is 204 with no receipt and no post when the flag is off closes it.
Where the HOLD is not. Both cards render and cannot shadow one another: approval-card, byo-handoff and gig-card are three disjoint payload.kind branches (V2MessageRow.tsx:444-452), and the decision branch above them is not kind-derived. The CSS is additive ahead of .v2-approval, uses existing tokens, and the invariant arm pins overflow-wrap: anywhere rather than break-word. The element catalogue is closed, the link validator admits only https github.com without credentials or port, and the 16-element budget bounds nesting depth as well as count — a deeply nested payload stops at depth 17. No GitHub-authored title, body or review text reaches the card or the agent-visible content. Three mutations reddened for their own reasons: dropping prUrl from the completion filter (1 red), removing the receipt release (1 red), and letting an unbound repository resolve to a pod (2 red). Suites, run one at a time: webhook 12/12, service 6/6, binding 1/1, limiter census 6/6, frontend gig card + layout invariants 221/221. helm lint clean for dev and staging. The tree was restored after every mutation.
Two notes for the press, not defects. The branch is behind main on exactly the two files its Helm resolution touches, because #2430 edited api-keys.yaml and values.yaml; git merge-tree is clean as text and I measured on that merged tree, but the resolution will be redone, and the prefix fix belongs in the same pass. Separately, the bind route lets an instance admin bind a repository into a pod they are not a member of. The card carries no GitHub-authored text, so I am not calling that a leak, but it writes into a pod whose members did not opt in, where the repo's other pod-scoped routes gate on membership — worth an explicit ruling rather than a precedent set in passing.

Gate on #2383 at exact head c87dc95 (TASK-311): HOLD — one blocking defect in the binding route; the webhook itself holds every property.
Blocking: the bind route does not require that the binder can see the pod. PUT /api/integrations/github-app/:id/gig-pod checks adminAuth, that the pod exists, and that its type is not agent-room/agent-dm. It never checks the binder against the pod. Probe, using the PR's own binding-test harness (admin caller, pods whose only member is another user):
| target pod | result |
|---|---|
another user's chat pod, binder not a member |
200, binding written |
another user's agent-admin pod (a personal pod type that 404s non-members on direct GET) |
200, binding written |
This matters because a binding is a durable write channel, not a read. Every later signed delivery posts a card into that pod as commonly-bot, and a merged PR auto-completes a matching task there. DMService.canViewPod does admit every admin, but its own comment scopes that bypass to read-only observability: "Write paths … enforce their own admin/membership rules." So the bypass doesn't satisfy this route.
Fix:
- Require
pod.membersto includereq.user.id, and return 403 otherwise. - Add
agent-adminto the refused types, next toagent-room/agent-dm. - Add test arms: an admin non-member binding a
chatpod gets 403 and writes no row; anagent-adminpod gets 400.
Everything else holds, and each part reddens when broken. Suites at head: 27/27 (webhook, binding, service, rate-limit separation); CI at this head ran all three new suites. Single mutations, one at a time, tests at head:
| mutation | result |
|---|---|
timingSafeEqual → length-only compare |
red: wrong-secret test |
GITHUB_GIG_ENABLED check removed |
red: gig-disabled test |
binding lookup ignores owner/repo |
red: indexed-lookup test |
| replay receipt not created | 4 red, incl. replay test |
| unbound early-return removed (receipt written for unbound) | 2 red, incl. unbound-silent test |
| DM-type refusal removed | red: binding test |
adminAuth removed from PUT |
red: binding test |
task-completion filter drops podId |
red: merged-PR test |
- Signature: HMAC-SHA256 is computed over the raw bytes, and the comparison is constant-time with a length guard. The raw parser is mounted before every JSON parser on this path, so bodies are never re-serialised before verification. The check runs before the flag, the parse, the binding lookup and the receipt.
- Task writes:
completeTaskForMergedPullwrites onlystatus, completedAtand one system update note. The filter is{ podId: <bound pod>, prUrl: <URL rebuilt from validated owner/repo/number>, status: pending|claimed }, so no field comes from payload text.Task.tsadds only an index.
Non-blocking:
X-GitHub-Deliveryis not covered by the HMAC, so anyone holding one signed body could replay it under a fresh delivery id and post a duplicate card. Task completion is idempotent, so a duplicate card is the only effect. This is standard GitHub webhook exposure; noting it, not asking for a change.- Owner and repo are matched case-sensitively against the payload's
login/name. A binding typed in different case is a silent no-op. Normalising both sides would remove that trap.

Wrap the new gig-pod handlers in try/catch. A rejected promise can crash the process.
Both handlers are async and have no error handling. The router is Express 4 (^4.18.2), which does not forward a rejected promise from an async handler to the error middleware.
A transient Mongo failure in any of these calls rejects the handler with no response:
Integration.findOnePod.findByIdGithubGigBinding.findOneAndUpdateGithubGigBinding.deleteOne
The same happens if new Types.ObjectId(String(req.user?.id)) throws.
The request then hangs. It also raises an unhandled rejection. Node 15 and later terminate the process on an unhandled rejection unless a handler is installed.
Every other handler in this file uses try/catch with a 500 response. Follow that pattern here.
Proposed fix (PUT shown; apply the same wrapper to DELETE)

Keep failed GitHub receipts retryable.
The route creates a permanent receipt before handleGithubGig completes. If the handler fails and deleteOne also fails, the receipt remains. A retry then hits the unique deliveryId or bodyHash index and returns 204 without invoking the handler. This can permanently lose the activity after the database recovers.
Use a durable receipt state or equivalent reclaimable claim. Mark the receipt completed only after the handler succeeds. Return 204 only for completed receipts. Keep failed or in-progress receipts retryable, with recovery for abandoned claims.

Separate webhook-secret provisioning from activation.
When githubGig.fromSecret is false, this block omits both the secret and GITHUB_GIG_ENABLED. The webhook verifies the signature with GITHUB_GIG_WEBHOOK_SECRET before it checks the enabled flag. As a result, App-wide deliveries receive 401 while the flag is off, rather than reaching the intended silent 204 path. Allow the backend to receive the secret while processing remains disabled, and test a signed delivery against that rendered configuration.

Re-gate on #2383 at exact head 280e470 (TASK-311): PASS. This answers my HOLD at c87dc95.
The blocker is fixed. The bind route now loads members, refuses agent-admin alongside agent-room and agent-dm (400), and returns 403 unless req.user.id is a member. I re-ran my original probe (admin caller; pods whose only member is another user) against this head:
| target pod | at c87dc95 | at 280e470 |
|---|---|---|
another user's chat pod |
200, binding written | 403 |
another user's agent-admin pod |
200, binding written | 400 |
| binding rows written | 2 | 0 |
Delta: 7 files, base unchanged at 71bd369d7. Suites at head: 28/28. CI is green (22 pass, 3 skipped), and the run at this head executed all three new suites. Each new piece reddens when removed (one mutation at a time):
| mutation | result |
|---|---|
| membership check removed | red: binding test |
agent-admin refusal removed |
red: binding test |
| webhook lookup not lowercased | red: indexed-lookup test |
| PUT owner/repo not lowercased | red: binding test |
| DELETE owner/repo not lowercased | red: binding test |
bodyHash unique index removed |
red: "a new delivery id cannot replay the same signed body" (isolated run; the head control is green) |
bodyHash hashed from the delivery id instead of the raw body |
red: replay test |
Both non-blocking notes from the HOLD are also addressed:
- Case: the route stores lowercased owner/repo, and the webhook lookup lowercases the payload's values. The card link and the task-completion
prUrlstill use the payload's own casing, which is unchanged behaviour. No rows predate this, because the gig has never been on. - Replay under a new delivery id: the receipt now stores the SHA-256 of the signed raw bytes under a partial unique index. A re-sent body with a fresh
X-GitHub-Deliverytherefore hits 11000 and returns 204, with no second post. The handler-failure path still deletes the receipt bydeliveryId, which releases the hash too, so a GitHub retry still processes. The index comes from mongoose autoIndex at boot; only one-off scripts in this repo disable autoIndex, and the collection is new, so it builds on first start.
Nothing else in the webhook changed. Every property I verified at c87dc95 still holds: the raw-body constant-time signature check, the flag, the exact binding, the unbound path writing no receipt, the rate-limit separation, and task writes scoped to the bound pod.

Rebase re-stamp on #2383 at exact head cf42251 (TASK-311): PASS. This carries forward my PASS at 280e470.
Only the base changed. The base moved from 71bd369d7 to 9ac61bdef. git range-diff 71bd369d7..280e470c 9ac61bdef..cf42251f shows one commit and one difference: the context line where the appended describe block lands in frontend/src/v2/__tests__/v2-layout-invariants.test.ts, which moved because main added the connector inactive-fold block above it. Every added line is byte-identical to the head I passed. No backend, model, route or test line changed.
Re-run at this head:
- Backend suites: 28/28 (webhook, binding, service, rate-limit separation).
- Binding probe, from the PR's own harness: an admin who isn't a member binding another user's
chatpod gets 403; binding anagent-adminpod gets 400; 0 binding rows are written. - CI: 22 pass, 2 skipped. The run at this head executed
github.webhook,integrations.githubGigBinding,githubGigService,V2GigCardandv2-layout-invariants.
Every property from my earlier PASS still holds.
