feat(web): dynamically render SCM and Log headers from package.json and omit web OAuth - #649

- Omit OAuth sign-in options and prompts on web where OAuth host support is unavailable (supportsOAuth: false)
- Dynamically render SCM and JJ Log header toolbar actions from package.json menu contributions (scm/title and view/title) filtered via when-clauses
- Support custom contributed icon jj-icon-workspace-add mapping to codicon unicode \ec58 (codicon-new-collection)
- Remove hardcoded reload button from Log header so only package.json actions are rendered
- Wire jj-view.manageAuth command in web commands with full quick-pick management flow
