Format the Event Issue Template Title and Bodies

Using the Issue Form Templates for creating Issue Events, we should be able to easily format and update the issue body and title from the information provided.

Here is an example format I think should work for us

Issue Title:

Here is the ticket we chatted about @gr2m. I didn't include the concept of location polls yet, as that could be an inhancement, and not sure we can get the details of an address from the forms yet.

Glossary

Neighborhood - A small hyper local area of Los Angeles that serves members of that area.

Event - An recurring date and time that members of a neighborhood can attend in person to meet and chat with each other

Location - The physical place for an event for members to meet

Champion - The volunteer(s) who manage a Lunch.js event.

I'm not sure if we need a table for the neighborhoods, I think we can just take the list from https://maps.latimes.com/neighborhoods/index.html? I think having a locations table and a events table should suffice.

I'm really excited to work on this but have to very much focus on my new gig right now. But I hope to find some time in between when I just need to work on something else, I'll prioritize work on lunch.js
