Add reconciliation operator actions

Summary

• Add control-plane reconciliation actions for claim, release, and manual-review transitions.
• Make Postgres reconciliation control actions mutate settlement state and write audit events in one transaction.
• Use canonical operator lease owners, tighten active settlement claim safety, and expose operator-aware controls in the ops console.
• Add proxy routes, frontend action wiring, Playwright coverage, unit tests, and live Postgres migration/release/atomic-action tests.
