feat: wire remediation gateway into CLI and Action - #16

Add OctokitRemediationGateway for draft PR creation from static mutations, required-check observation, and draft promotion. Expose techdebtter remediate and Action phase remediate with phase-scoped contents/pull_requests write permissions. Co-authored-by: Derek Palmer <derek@derekpalmercreative.com>
